Typical Format

1
Initial Consultation (first session)
This 1-hour session gives us a chance to get a clearer idea of the individual’s personal history and current challenges and gives individuals and/or caregivers a chance to voice their concerns, express their goals, and ask questions. This information helps guide the selection of appropriate assessment tools and informs the development of a tailored evaluation plan, which is discussed during the session.
2
Assessment Sessions
Include standardized questionnaires, interviews, and/or cognitive assessments that are completed over two (or more) 2 to 3 hour sessions, depending on the referral questions.
3
Scoring and Report Preparation
All the collected information is scored, analyzed, and integrated into a comprehensive written report. The report is available approximately* 2-4 weeks after the final testing session.

*Please note: Delays in receiving forms/questionnaires will extend the timeline for completing the report.
4
Feedback Session
This 1-hour session gives us an opportunity to meet with the individual and/or caregivers to review the results, provide a clear explanation of any diagnoses (if applicable), answer questions, and discuss individualized recommendations for support, intervention, and next steps.
